A Guide to Pain Control for IUD and Endometrial Biopsy Procedures

What to Expect & How We Keep You Comfortable

At Gabrielson Clinic for Women, your comfort matters. Historically, discomfort during women’s health procedures has not always been fully acknowledged or addressed within the medical system. At the same time, while social media often highlights difficult experiences, the majority of patients tolerate IUD insertion well and report that it is easier than they expected.

Today, new 2026 guidelines from the CDC and American College of Obstetricians and Gynecologists (ACOG) recommend that providers proactively discuss and offer pain control options. We believe informed patients feel more confident and have better experiences. This guide explains pain control for IUD insertions and endometrial biopsy sampling.

Understanding IUDs

An intrauterine device (IUD) is a small, flexible, T-shaped device placed inside the uterus to prevent pregnancy. There are two main types:

Hormonal IUD

Releases a small amount of the progestin hormone levonorgestrel to:

  • Thicken cervical mucus
  • Block sperm
  • Reduce heavy periods and cramping
  • Help treat endometriosis and pelvic pain

Hormonal IUDs last up to 8 years.

Copper IUD

This device is wrapped in copper, which prevents sperm from reaching the egg. It works immediately and can last up to 10 years. IUDs are more than 99% effective. Once placed, they require no daily action.

Does IUD Insertion Hurt?

For most patients, IUD insertion takes about 90 seconds to 3 minutes.

You may feel:

  • Cramping (like strong period cramps)
  • Pressure
  • Brief sharp discomfort

Pain levels vary. Some patients feel mild discomfort, while others feel more intense cramping. Anxiety can increase how pain feels, which is why conversation and preparation matter.

Pain Control Options for IUD Insertion

We offer several types of pain management. These fall into three categories: local, systemic, and comfort-based methods.

1. Local Anesthetics (Numbing the Cervix)

These work directly at the procedure site.

Paracervical block offered by our own OB/GYN team

  • Lidocaine is injected around the cervix
  • Considered one of the most effective ways to reduce procedural pain

This option is especially helpful for patients who want stronger pain control.

2. Systemic Medications (Whole-Body Relief)

These medications affect your whole body.

NSAIDs (nonsteroidal anti-inflammatory drugs)

  • Ibuprofen (Advil, Motrin)
  • Naproxen (Aleve)

Taken 30–60 minutes before the procedure. These help reduce cramping afterward.

3. Comfort & Non-Medication Techniques

Simple tools can make a big difference:

  • Heating pads
  • Deep breathing techniques
  • Music
  • Support person present
  • Distraction methods like guided coughing

Ultrasound-Guided IUD Insertion

Ultrasound technology improves both comfort and accuracy.

What Is Ultrasound Guidance?

A transabdominal ultrasound probe is placed on your lower abdomen during the procedure. It creates a real-time image of your uterus.

This allows your provider to:

  • See the angle of the cervix
  • Measure the depth of the uterus
  • Guide the IUD precisely into place
  • Confirm proper positioning immediately

Why Use Ultrasound for IUD Placement?

Ultrasound-guided IUD insertion can:

  • Reduce discomfort
  • Avoid repeated attempts
  • Help with tilted (retroverted) uteri
  • Improve placement accuracy
  • Lower risk of perforation
  • Confirm proper positioning instantly

Your Comfort Is Part of Quality Care

With clear counseling, modern pain control options, and ultrasound guidance when needed, these procedures are safe, quick, highly effective, and very manageable for most patients.
