How to Transfer Your Care
Getting started is easy. In most cases, you only need to complete one form, and we handle the rest.

Step 2: Complete a Release of Information Form
To get started, complete the Release of Information (ROI) form so we can request your records.
Step 3: We Request Your Medical Records
Our team will contact your current medical provider to collect:
- Medical history
- Lab results
- Imaging (such as ultrasounds)
Step 4: Begin Your Care
Once your records are received, we will:
- Review your history
- Schedule your visit
- Create a personalized care plan
How Long Does It Take?
Most transfers are completed within a few days, depending on how quickly we receive your records.
